Only while your tour guide license is still valid. Open Services, then Lost or damaged card. If the license has expired, renew it first.
A letter of police for the lost card. Choose Lost on the form. The damaged card photo is not required for a lost card.
A photo of the damaged card. Choose Damage on the form. The police letter is not required for a damaged card.

Notes

Sign in to the Tour Guide portal and open the matching item under Services. The form shows only the attachments for that application.
